Evidence of Insurability/Insurance/Applicable Licenses
USSI hereby certifies that we have, and will continue to hold, the required and necessary insurance
coverage, valid licensure, and applicable business licenses as needed to perform the duties and assume
the responsibilites required of a Supplemental Staffing Agency. Some of these include, but are not
limited to, General Liability, Workers Compensation, and Professional Liability. We further attest that
this insurance and licensure are in compliance with any and all local, state, and federal laws and
regulations pertaining to typical and standard licensing requirements.
