Registry Qualifications:
United Staffing Solutions, Inc. employs only the most highly-trained, and qualified Healthcare Professionals
available in the industry today. We pride ourselves in providing only the most skilled personnel for our Client
Facilities, and have developed certain clinical and operational "Policies and Procedures" in accordance
with our Corporate standards that must be met, and adhered to, by all USSI employees. It is mandatory that all
of our HealthCare Personnel strictly follow our policies regarding Minimum Employment Requirements for clinical employees;
and for their particular area of expertise. One hundred percent (100%) of any of the staff that we recommend to
our Client Facilities are try employees of United Staffing Solutions, Inc. They have gone through the full
application process which includes, but is not limited to, completing all relative Skills Check Lists, NLN Test Exams,
Medication Exams, Job Descriptions, and Safety and Health Modules. Additionally, they are required to furnish any
and all necessary licensure, evidence of Health Screens and/or Physical Exams, a Tuberculosis Skin Test and/or Chest X-Ray
, a Rubella and/or Titer Vaccination(s), and a Hepatitis "B" Antibody Test and/or Vaccine or a "Declaration
of Declination". Eligibility for hire is subject to our performing all applicable reference checks on their
performance and clinical history prior to our placing them into our Client Facilities. We also perform mandatory
drug testing on any new applicants, random tests as desired or needed, an/or as may be required by our Client Facilities
from time to time.

Professional Qualifications:
USSI required that all necessary and appropriate certificates, credentials, and licensure as required by Federal, State,
County, and Local law are presented to our office at the time the prospective employee fills out their initial
application; and that copies are kept on file in our office in the appropriate applicant file. We verify all
information and make sure that items presented are the true possession of the carrier, and that they remain current.
Our Director of Nursing, along with our in-house office staff, regularly maintains and monitors HealthCare
Professionals files for compliance and outdated materials. It is mandatory that any clinical USSI employee is reviewed
and given a performance evaluation by our D.O.N at least annually or sooner, based upon need, at the Company's sole
discretion. Additionally, we also procure evaluations from Client Facilities on employees periodically as necessary or required.
As part of our Quality Assurance Program, all complaints or DNR or DNU reports are researched and investigated thoroughly;
and appropriate actions and recommendations, including dismissal, are implemented if deemed necessary and appropriate. We
have developed and utilize strict and inflexible "Policies and Procedures" in terms of acceptable clinical employee behavior,
work ethic, and level of performance; and ensure that all USSI employees are in complete and total compliance with all
JCAHO guidelines, in addition to our Corporate "Policies and Procedures" and Standards of Conduct.
Additional Specialty Qualifications:
In addition to the qualifications previously detailed above, there are some additional specialty requirements for
various personnel as follows:
All USSI Healthcare Professionals must possess a valid and current American Heart Association Cardio Pulmonary
Resuscitation (CPR) card.
All USSI Emergency Room and Intensive Care Unit Registered Nurses (ER and ICU RNs) must possess an American Heart
Advanced Cardiac Life Support (ACLS) card.
All USSI Labor and Delivery Nurses (L&D RNs) must possess and maintain a Certificate of Completion of a Fetal
Monitoring Class and, in most cases, an Advanced Fetal Monitoring Class.
