In regard to Professional HealthCare Staffing, United Staffing
Solutions, Inc. (USSI) is available to assist our Client Facilities with Per-Diem and Travel Nurses, Specialty
Nursing, and Support Services on an "as needed" basis. We will proudly commit and continue to adhere to the following
standards and criteria:

A. USSI will be "On Call" to any or all Client Facilities as contracted. Our offices do not
use any answering services or automated answering devices, so our Client Facilities will always speak with an
employee of USSI; who can understand the needs and urgency of any given situation.
B. The Director of Nursing at USSI is Nick Kosareff, R.N., who is a California Registered Nurse. Nick is available
24 hours a day, 7 days a week for nursing support; both in the field and in area hospitals. He is always available
for any requested consultations or counseling needs of our Client Facilities or HealthCare Professionals; should
the need arise.
C. It is part of USSI's "Policies and Procedures" that all nurses undergo extensive screening by a Registered Nurse.
All employees who work for USSI must all have proper credentials for their specialty, and a minimum of one (1) year
recent acute care experiance in their specific area of expertise. All nurses are required to meet, and often exceed
, JCAHO requirements and standards; in accordance with our own guidelines.
D. USSI requires a pre-employment physical and pre-employment PPD and/or chest x-ray reviewed by a physician or his/
her designee. After the initial pre-employment information is submited to USSI, we insist upon keeping this information
current at all times; thus ensuring that all USSI Healthcare Personnel are not only JCAHO compliant, but Facility compliant
as well.
E. United Staffing Solutions, Inc's Corporate Office is located in Culver City, California. Our local telephone number is

Office is home to the President & CEO (Parvez Gondal, M.D.), the company's Administrator, our Los Angeles Per-Diem Staffing
team, and all Accounting personnel. It also accommodates and supports our Travel Nurse Recruitment Division as well.
All relative Healthcare Professionals credentials, certifications, and other pertinent documentation is kept in the Corporate
Office; and is available for review upon your request at anytime.
